just run out and fill out the LLC form- child's play.
Whether you should have an LLC or some other form of organization? Who cares?
Should you be a "disregarded entity" What's that? Ask that child who is playing.
Whether you would be better off spending your organizational filing fee and entity taxes on insurance? Child's play.
The format for your Operating Agreement? (Hint: the state provides a one page form articles of organization. It does not provide an operating agreement) What's an operating agreement? Child's play
Future expansion? Investors? Partners? if not now, in the future. More Child's play.
Licensed occupation? Can you even have an LLC? No doubt even that child could figure that out. (Note: accountants routinely miss this one.)
I have a CT LLC and had my lawyer set it up for me. I didn't try to figure out whether I needed him or not as the fee was pretty nominal. As you probably no, you need at least two "members" in an LLC. Mine was really a sole proprietership, so we set it up with me as a 99% owner and my wife as a 1% owner.
You should probably talk with an accountant though. There are ins and outs based on your income and assets, etc. Once you know what sort of corporate entity is best for you the formation and filing stuff can be done on the web.
Never skimp on professional services. If you can't afford to do it right, you're doing something wrong. I would always run it by an attorney. In the end you're much better off.
LOL!
Also, it's easy to setup your own federal tax id# do that.
As stated above, getting your EIN is easy.

Just fill out the forms on the government website and save your self the money. Hiring a lawyer on day one of your business is a bad sign.

That depends if you are working with contracts or not.
