Where "Naked and Afraid" usually has athletic-type survivalists a spin-off would be more realistic. My idea... You get a couple of Grossly obese people and lock them up in a vegetarian health food store for 21 days. "Survivalists" would have to adapt and overcome a foreign environment and endure hardship. This show would be informative and serve provide social service for healthy lifestyles. It could be called "Naked, Fat and Hungry"

A twist: Wouldn't have to necessarily be a health food store. Just an apartment or whatever. Do a "food drop" each day, or every other day, with slightly less food than might humanely be appropriate for an average person, but you'd have the overweight people ... and they'd have to "work it out" on how the food would be divvied up.
Every 3-4 days or so (could be on whatever schedule you'd choose), decrease the amount of healthy food in the "drop." Maybe start with 10 people. No one gets voted out. They just have to be naked the whole time. Oh, and no showers. LOL
