NFT: Just diagnosed with testicular cancer

Hey guys. Was diagnosed lastweek with testicular cancer. 43 years old. Noticed one of my boys doubled in size past few months and ultrasound confirmed it. I had surgery yesterday in Saratoga springs to remove the testicle..
I’m just wondering if anyone has experience with this? I know it’s kind of rare cancer but very treatable. But I also heard you want to find someone who sees it a lot.
Right now my care team is in Saratoga. I have not seen an oncologist yet. But I do know ct scan was good and tumor markers negative.
I have been thinking of forgoing the local care team and going to Sloan memorial? I assume they probably see way more of this then anything upstate? Guess I’m just wondering if this is good idea to go to a bigger place like Sloan instead of the local folks in Saratoga/Albany?

Had one removed and has been cancer free for over a decade. With all serious procedures I would advise anyone to try and go to the best facility possible. If you have access to a different hospital that specializes, go for it.

I am praying for you. It thankfully sounds as if you are on the road to recovery, but this must be extremely frightening. I agree with what everyone else has said, namely that you should go to the best place possible for treatment. Good luck. Best wishes.

...is don't fuck around and go to the best doctor/hospital for cancer that your insurance will pay for, even if it means going to NYC (i.e. Sloan Kettering).

for a fast recovery.

As a PSA, my brother was diagnosed with breast cancer several months ago. Prognosis is great, surgery to remove it, he's been going through some preventative rounds of chemo in case there were any miscroscopic cancer cells left in his system as he's still very young to prevent trouble down the road. Chemo is a bitch. The first few rounds really took a toll on him but he's in a less intense chemo cycle now until he finishes in a couple months, thank God.

Guys, check yourselves. Not just your balls but also your nipples.

Had a friend who had this in his 30's. He was cancer free but he couldn't handle the loss...

I second the suggestion to see the best doctor your insurance pays for. The national cancer centers are usually a good choice.

Sometimes going to the biggest operation with their Ivy League doctors isn't the best choice over a smaller operation with less heralded doctors. Bigtime doctors can be harder to reach and are more likely to delegate to interns and other staff, it can be less personal, and it's easier to get lost in the sauce. I'm not saying this is the case between Sloan and your Saratoga team, but I wouldn't just assume you will get better care in the big city.

on the positive side, I have read this is maybe the most "curable" form of cancer.

Also, I'm not an expert in this, but if you don't yet have children and do want them, maybe look into banking your sperm. I don't know if the cancer impact virility or ability to have kids. Your doctors obviously will advise you here.

My brother has used Sloan for a number of years. He says sometimes the doctors are very rushed due to the large number of patients seen, but that the nurses are wonderful and always return calls the same day

I had the same cancer when I was younger at 24. I went to Sloan to have mine treated but mine was much worse and had spread to my kidney which I ended up having removed. I was also on a newer chemo regimen to treat. I was only the 8th or 9th person to have it at the time. I am thankful to this day I went to Sloan over anyone else. I would try and get in with them.

But plenty of other hospitals are very satisfactory for this these days. Especially if regular treatments are involved, you may prefer closer to home.

But an opinion from MSK will be worth its weight in gold, even if you choose to go elsewhere. Peace of mind is very important. Good luck.

he had some kind of what was then an experimental process at one of the top Boston hospitals (I believe Mass General). Lost both of his testicles (they saved semen) and now has two gorgeous daughters from the saved semen. I think he had some kind of stem cell process where they wiped out his immune system and rebuilt it. Sorry, I am hazy on the details, but I know it was on the cutting edge because he was so young.

My point is I would investigate Boston if I were in your position.

I have a friend that had the same thing and recovered fully with no reoccurrence, and that was years ago now.

what was up with his nipples if you dont mind me asking? I knew men can get breast cancer, but didnt know symptoms were nipple related at all

I'm four years removed from treatment for colon/rectal cancer. I can't speak to the particulars for your treatment but can help when it comes to dealing with chemo.

I had an "interesting" case. You never want an interesting case of cancer. Not only is it more complicated to treat but every doctor and intern wants to examine you. And with rectal cancer that is not fun. But what I did find is that the top oncologists do help out other car givers. My case was put up in front of a group of doctors from sloan, mayo and the Cleveland center. They discussed my case and came up with a treatment plan that was given to my locally.

I wouldn't recommend going far for the chemo treatments. Once the type of chemo and the dosage is determined, the care is pretty simple.

Sloan also has treatment centers located in other places than nyc. There might be one closer to you.

Hard, noticeable lump on one nipple. Had it checked out pre-covid. A couple doctors looked at it and were reassuring that it wasn't a big deal but he to get it needle biopsied. Then covid hit. He wasn't able to schedule it for awhile but found out in November it was in fact cancer.

They can share data such as scans and tests and such electronically, and can also review the pathology slides. What you need from the referral center is a plan of care - the therapy, especially chemotherapy, can be administered locally in most cases.

A similar option would be to seek a second opinion regarding therapy at a big center (Sloan-Kettering to your south, Dana Farber to the east). They could communicate and collaborate with your local oncologists.

Testicular cancer is very treatable. Good luck!

My friend's brother had bladder cancer. He opted for local treatment after the doctors at MSK recommended removal. He died a few years later. The best hospitals are the best for a reason.

Not necessarily. MSK is useful for rare/unusual cancers, but they overtreat garden variety ones. I know people who have died as a result.

Well bladder cancer is not especially rare and MSK wanted to treat it aggressively, what you might call overtreat, and he decided not to take their advice. And it cost him his life.
