May have burned his toes in house fire. That would explain why he wanted the fentanyl. The access to this very dangerous pain killer is a huge problem.

There’s been a huge spike in overdose deaths from fentanyl mixed in cocaine over the last five years. A lot of recreational cocaine users use strips to test if cocaine has been accidentally tainted with fentanyl. Trace amounts can be lethal.
The cause is usually accidental contamination from equipment when drugs are produced and recut along the supply chain. Obviously there isn’t a quality control process that’s regulated when coke is cut in Colombia, exchanging hands through the Caribbean, and into the hands of street dealers in the US.
The most likely and very sad explanation is he thought he was doing recreational cocaine, and he inadvertently was doing something tremendously dangerous.
